Senior Activity Sheet
The Senior Activity Sheet is an important form for seniors to complete. Seniors can pick up a copy of this form in the guidance office. Seniors should carefully complete this form. It is a place to list all activities and involvement. Please be neat when completing the activity sheet. Typing the form or printing in black ink is preferable.
Activities are not listed on the transcript. The activity sheet is used by teachers and counselors for letters of recommendation. College admissions offices and scholarship donors often request this information. This form will be used as a supplement to applications you complete. A copy of the form will be kept in the Guidance Office throughout the year.
